Appointments

We know you have many choices when choosing a Dentist in Union Grove, WI so we have made requesting an appointment a simple process via our Web site. If, for any reason you cannot keep a scheduled appointment, or will be delayed, please call us as soon as possible.

Insurance and Billing

We are happy to file insurance claims on your behalf and accept most insurance plans that allow you to seek treatment with a dentist of your choice.  

PPO Plans: Always verify with your dental insurance plan's provider directory to see if we are in your network!  The Family Dentists, S.C. does not participate in Health Management Organizations.

We do participate with the following PPO plans:

Cigna PPO, DBP (Dental Benefit Providers), Delta Dental (Premier Provider), DHA (Dental Health Alliance - Aetna/Assurant), Humana, and the Stratose/Maverest Network (which can include Ameritas, Anthem Complete, Dearborn National, Guardian, Metlife PDP Plus & FEDVIP, Lincoln Financial Group, and Principal). 

Again we stress that you check with your dental insurance plan's provider directory to ensure that we are listed as a provider in your network!

We will be happy to file your insurance claims for you.  As a courtesy, we try to obtain insurance estimated co-payments, maximums, and deductible information; but due to the complexities of constantly changing plan benefits, it often makes it difficult to obtain accurate information.  It is therefore the responsibility of the patient to obtain any information they may have in question.  We accept checks, cash, MasterCard, Visa, Discover, American Express, and CareCredit (a deferred interest credit card option for patients wishing to extend their payments over time) - Please see the front office for details.

At the same time, we realize that you are an individual with preferences of your own.  If you have any emergency situation that needs attention, just let us know and we will take care of that need first.  If not, we will thoroughly examine your mouth, take necessary x-rays and let you know what your oral condition is.

We do ask that all co-pays and co-insurance are paid as services are rendered.  We are able to offer a flexible payment plan with CareCredit with no-interest financing for up to 12 months.  Please see our front office for details.
