Appointments

We know you have many choices when choosing a Dentist in Union Grove, WI so we have made requesting an appointment a simple process via our Web site. If, for any reason you cannot keep a scheduled appointment, or will be delayed, please call us as soon as possible.

Insurance and Billing

We file most traditional insurance plan claims, contact our office to verify acceptance of your plan. The Family Dentists, S.C. does not participate in Health Management Organizations; however, we are a Delta Dental Premier provider, Humana Network provider, and a Cigna PPO network provider.  Newly we are happy to announce that we are preferred providers for Dental Health Alliance who features Assurant PPO effective 2/1/2016; United Concordia PPO effective 3/10/2016, and Aetna PPO that will be effective on 5/1/2016.

We will be happy to file your insurance claims for you.  As a courtesy, we try to obtain insurance estimated co-payments, maximums, and deductible information; but due to the complexities of constantly changing plan benefits, it often makes it difficult to obtain accurate information.  It is therefore the responsibility of the patient to obtain any information they may have in question.  We accept checks, cash, MasterCard, Visa and CareCredit (a deferred interest credit card option for patients wishing to extend their payments over time).  Please see Jean or Mary for details. We are happy to file insurance for your reimbursement as long as you are free to choose your own dentist.

At the same time, we realize that you are an individual with preferences of your own.  If you have any emergency situation that needs attention, just let us know and we will take care of that need first.  If not, we will thoroughly examine your mouth, take necessary x-rays and let you know what your oral condition is.
